Yup, everything is back to normal. I don't know how the swap partition has disappeared, most probably I removed it myself and I don't remember. I guess everything that messes with the ESP uses clr-boot-manager so it isn't some weird service/daemon going mad and deleting partitions. Again, thanks for your help and sorry for wasting your time.

Interesting, there is no such partition. I'll post logs after I reboot.
