I was just reminded to mention that this all worked correctly on kernel 4.9, and then stopped working.
I updated the kernel to 4.14 this morning while trying to make this work.

I am setting the "Initial Display Output". If I set "IGFX" then the onboard graphics card works (with it's one monitor), if I set it to (PCIe Slot 1) then the Radeon card works (with its two monitors)
